definition:
hypersecretion of hydrochloric acid in the stomach
etiology:
The causes include physical or mental/emotional stress, caffeine (coffee, chocolate, cocoa, black tea), hypoglycemia, and Zollinger-Ellison syndrome (gastrin-secreting pancreatic tumor)
signs and symptoms
signs and symptoms:
heartburn
peptic ulcer disease
gastritis
lab findings:
(+) Heidelberg pH capsule analysis for acidity
increased serum gastrin (with Z-E disease)
(+) endoscopy for ulceration (possibly)
(+) for hypoglycemia (possibly)
course and prognosis
Zollinger-Ellison syndrome is a serious disease that requires careful treatment and close follow-ups. 60% of the pancreatic tumors are malignant.
In other causes of hyperchlorhydria, peptic ulcers may occur if the hyperchlorhydria is left untreated, and gastritis may develop and become chronic.
differential diagnosis
Hypergastrinemia can be found in:
pernicious anemia
renal insufficiency
chronic gastritis
pheochromocytoma
marked intestinal resection
Also need to differentiate:
esophagitis
hiatal hernia
gastroesophageal reflux
peptic ulcer disease
